No. All seven US Philippine posts capture your photo digitally on-site. Multiple posts explicitly state "do not bring passport pictures." Your photo and fingerprints are taken as part of biometric enrollment.

No. NY, Chicago, and LA all list eyeglasses/shades as rejection reasons. LA instructs applicants to remove eyeglasses and contact lenses immediately before capture. No routine medical exception is published.

Some posts explicitly allow a "medium" smile with no teeth showing. An overly wide or toothy smile will be rejected. The safest approach is a neutral expression with mouth closed.
